3) Send your preferred appointment times and dates by e-mail to the scheduler, Rubin, so that he can schedule your appointment.

4) You will be notified of the final time and date of your appointment.

5) If you do not have e-mail access, you may call 323-219-2469 x301 and leave a message with your preferred dates and times.

For established patients:
